Your answers to the questions on this medical form will be kept strictly confidential by GRRRLTRAVELER. It may be used to determine your suitability for travel or to provide emergency medical assistance.

Our adventure tours are often designed for travelers who are in relatively good health for their safety as well as that of their fellow travelers. Tours may visit remote locations with few or no high-tech medical facilities. Although a medical emergency situation is highly unlikely, should one arise, we want to be prepared to assist you with the essential knowledge.

After submitting the form to GRRRLTRAVELER and/or Tour Operator, you must contact them as soon as possible if your physical or medical condition changes, or if your answers to the questions in this form changes.

If you do not disclose a condition, infirmity, injury, or ailment herein and are subsequently deemed to be unfit for expedition travel due in whole or in part to such condition, infirmity, injury or ailment, GRRRLTRAVELER shall have the right to remove you from the tour with no refund or compensation payable.

What happens if I don’t complete this medical form?

In the event you are unable or refuse to complete this medical form for any reason by the final payment date as specified in our terms and conditions, GRRRLTRAVELER reserves the right to consider your booking cancelled as of that day and applicable cancellation penalties will apply.
